What does PCOS mean for your periods

It is not just one problem: PCOS is defined by a pattern of signs that include menstrual irregularity, higher androgen (male-pattern) hormones, and certain ovarian features. In terms of bleeding, the characteristic patterns are:

  • Long cycles, commonly 35 days or more, sometimes months between periods. This is because ovulation happens rarely or not at all, so the uterus does not shed on a monthly schedule.
  • Oligomenorrhea, which means infrequent periods, and amenorrhea, which means no periods for three months or longer when not pregnant or breastfeeding.
  • Heavy or unpredictable bleeding when it does happen. Without regular ovulation, the lining of the uterus can build up more and then come away in a heavier bleed or sudden irregular spotting.

Mechanism in plain words: menstrual bleeding requires a coordinated hormone rhythm. The brain (pituitary) releases follicle-stimulating hormone and luteinizing hormone to grow an egg and trigger ovulation. When ovulation happens, progesterone stabilizes the uterine lining then falls and causes a predictable bleed. In PCOS, that ovulation signal is weak or absent, so progesterone is low, the lining thickens for longer, and bleeding becomes erratic or heavy. Who gets this pattern: PCOS is common, affecting an estimated 6 to 12 percent of people assigned female at birth of reproductive age depending on the criteria used. Anovulation and endometrial buildup

Anovulation means no ovulation. Without the luteal phase progesterone that follows ovulation, the uterine lining keeps thickening. When bleeding finally occurs it can be heavier or longer than a typical period. Long gaps between periods increase the risk of irregular heavy bleeding and, over time, abnormal changes to the lining.

Variable ovarian response

Some ovaries produce many small follicles visible on ultrasound. The term "polycystic" means many small cyst-like follicles, not actual cysts that need removal. These follicles often fail to progress to a mature egg. Ultrasound appearance can change over time and is only one diagnostic piece.

Weight, stress and lifestyle

Weight changes can shift insulin and sex hormone balance fast. Gaining weight often worsens insulin resistance and menstrual irregularity, while modest weight loss (5 to 10 percent) can restore more regular ovulation for some people. Chronic stress and extreme exercise or eating patterns can also disrupt cycles, so look at the whole context.

PCOS symptoms beyond periods

PCOS is more than irregular bleeding. Typical symptoms include:

  • Irregular periods: the most common reason people seek care.
  • Excess hair growth (hirsutism): coarse hair on the face, chest, or back due to higher androgens.
  • Acne and thinning scalp hair: acne that is persistent or worsens after puberty, and male-pattern hair loss in some.

How is PCOS diagnosed: the three criteria explained

Doctors diagnose PCOS using one of three internationally accepted criteria sets; the most used are the Rotterdam criteria. Under Rotterdam, you need two of three features:

  • Irregular or absent ovulation: evidenced by missed periods or very long cycles.
  • Clinical or biochemical signs of hyperandrogenism: visible signs like excess hair or acne, or lab tests showing higher androgen levels.
  • Polycystic ovarian morphology on ultrasound: many small follicles seen on pelvic ultrasound.

Diagnosis requires excluding other conditions that mimic PCOS, such as thyroid disease, high prolactin, late-onset congenital adrenal hyperplasia, and certain medications. The process is a rule-in and rule-out routine rather than a single definitive test.

What tests to expect

Blood tests commonly include:

  • Pregnancy test, to rule out pregnancy before assuming amenorrhea.
  • Hormone panel, often including total testosterone or free androgen index, thyroid-stimulating hormone (TSH), and prolactin. Some clinicians check 17-hydroxyprogesterone to exclude adrenal causes.
  • Metabolic tests, fasting glucose, hemoglobin A1c, or an oral glucose tolerance test if insulin resistance is suspected.

Pelvic ultrasound looks at ovarian size and follicle number. A transvaginal ultrasound gives the clearest image, but a transabdominal ultrasound may be used in virginal adolescents. Ultrasound findings vary and must be interpreted with symptoms and labs.

Timing matters: some androgen tests are best done in the morning, and if a person is on hormonal birth control the results may be altered, so clinicians may request stopping certain medications before testing when safe and appropriate.

Blood tests and what the numbers mean

Androgen levels: Testosterone can be mildly elevated in PCOS, but lab reference ranges vary. Many people with PCOS have values near the upper limit of normal rather than massively high levels. Biochemical hyperandrogenism supports the diagnosis but is not required if clinical signs are clear.

Glucose and insulin-related tests: Fasting glucose and hemoglobin A1c screen for diabetes risk. An oral glucose tolerance test is more sensitive for detecting insulin resistance in PCOS, especially when fasting glucose is normal but risk is high.

Thyroid and prolactin: Thyroid disorders and high prolactin can cause irregular periods, so clinicians usually screen for them to exclude other causes.

Clinical practice varies, but the goals are clear: confirm ovulatory dysfunction, look for excess androgens, and assess metabolic risks that guide long-term care.

Managing periods with PCOS: general approach

Treatment has three common goals: establish regular bleeding to protect the uterine lining, reduce androgen-related symptoms if bothersome, and address metabolic risk. Choices depend on whether you want to become pregnant.

Combined hormonal contraceptives (pill, patch, ring) are a common first-line option to regulate cycles and reduce androgens. They menstrualize you by providing estrogen and progestin together, which makes bleeding predictable and lowers ovarian androgen production. Metformin is a diabetes medication that improves insulin sensitivity and may restore more regular cycles in people with insulin resistance. Anti-androgen medications like spironolactone can reduce unwanted hair and acne but require contraception when used because of potential teratogenic effects. Discuss risks and timing with your prescriber.
